schedule an appointment online. Once you have scheduled
an appointment, move on to step 2. |

| Step 2 - Complete our client information forms: |

| The information you provide here will be used by
our office to determine the best options available for your situation.
You will be required to provide biographical information (name, address,
etc.) and information about your debts, assets, income and expenses. Please have all specific information about
your debts handy while completing the forms (creditor names, billing addresses, account numbers, etc.). |

| Step 3 - Consult with one of our attorneys: |

| Meet with one of our attorneys to discuss your options,
provide additional information, and review your final paperwork prior to filing. |

If possible, please bring
the following documents to your first appointment:

Copies of all pay stubs or payment advices you have received within the last 60 days. |
|
Copies of your State and Federal tax returns for the most recent tax year. |
